Making An Appointment

Before your first visit you will be mailed an information packet that includes a health assessment form. Please complete the health assessment in advance as it is critical in helping us to gain a better understanding of your medical condition, and carry it to your appointment.

If we cannot mail you a packet before your visit, we ask that you please print the forms to the right and complete prior to your visit.

Bring To Appointment

1. Completed Health Assessment Form
2. Any Relevant X-Rays (films on disc are superior to reading films over the internet)
3. Any Pertinent Medical Information
4. Insurance Card
5. If Work-Related, Bring Case Manager's Name & Phone#
