If you have filed a worker's compensation claim with your employer involving your treatment at McLean County Orthopedics, we will send the bills to your employer or to your employer's work comp insurance. You must complete and sign a "First Report of Injury" through your employer in order to file a claim. We will need your employer's name, address, and phone number and the date of your injury. If there is no injury date, we need the date you put on the "First Report of Injury" as the beginning of the claim. Please be aware that this date needs to be the same as or prior to your first treatment date in order for the first treatment to be covered under your work comp claim.
Please inform us if your claim is denied at any point and we will bill your group insurance. We will need your group insurance information from the beginning of your treatment even if your work comp claim has been accepted.
If you go to litigation at any point for a denied work comp claim, please inform us of your attorney's name and phone number. We will bill your group insurance while the claim is in litigation and collect the co-pay from you at each visit. We will not bill you for any coinsurance or deductible while the claim is in litigation. You will receive a statement of the account balance from us every 90 days to keep you informed while your claim is in litigation. This will not be an attempt to collect payment from you during the litigation. Please keep us informed of any changes in the status of the litigation.
